For your convenience, and to expedite the check-in process, the following lodging rental agreement forms are available for you to review, initial, and sign. Please submit form(s) upon check-in.

Group Campsites with guest lists must also follow instructions below for Special Function Forms.

GUEST PET POLICY AGREEMEN

Once you have selected a site for your next celebration, a few more steps are necessary to finalize your event. For groups of ten or more people, the following MUST be completed and submitted no later than five working days prior to your event.

1. SPECIAL EVENT PROGRAM MEMORANDUM

Please REVIEW the Special Event Memorandum.

2. SPECIAL FUNCTION AGREEMENT

Submit a completed Special Function Agreement in-person to Security Forces at the Base Defense Operations Center (Bldg. 220) NO LATER THAN 5 days prior to your event. Any submissions WITHIN the 5-day window will not be accepted. *Note: Due to fire code restrictions Special Functions Agreements for events in the cabin/condo area will not be accepted.

3. GUEST LIST

After submitting Step 2, and having been properly vetted and verified, you will receive a DoD Safe Link to upload your Guest List to Security Forces NO LATER THAN 5 days prior to your event. You MUST use the DoD Safe Link to submit your Guest List. Sending your guest list via email will result in your guest list being rejected. Note: The file is an Excel spreadsheet. ALL information is required for guests ages 16 and older. DO NOT remove headers or delete cell information.

4A. CONFERENCE ROOM RENTAL AGREEMENT

Submit a completed Conference Room Rental Agreement to Reservations (Front Desk) for Conference Room rentals NO LATER THAN 14 days prior to the event.

4B. PAVILION-DECK RENTAL AGREEMENT & CHECKLIST

Submit a completed Pavilion-Deck Rental Agreement & Checklist to Turtle Cove for Outdoor Deck/Pavilion Rentals NO LATER THAN 14 days prior to the event.

5. Wedding Ceremony Agreement

6. OUTDOOR RECREATION SAFETY AGREEMENT

Submit a completed Outdoor Recreation Safety Agreement to Turtle Cove.

7. EQUIPMENT RENTAL WAIVER

Please submit (in-person or email: turtlecove@bellowsafs.com) a completed Equipment Rental Waiver to Turtle Cove if you wish to use the equipment.

All previous versions of the above forms will not be accepted.

Guests ARE subject to a criminal background check prior to access to the installation. Guests with unfavorable information WILL BE DENIED access to the installation. Foreign National Guests will NOT BE ALLOWED base entry unless/until they have been properly vetted through Security Forces (30 days prior.)
