SPECIAL EVENT CHECKLIST MUST BE SIGNED OFF AND TURNED IN AT LEAST 1 WEEK PRIOR TO YOUR EVENT. FAILURE TO ABIDE BY THESE REQUIREMENTS,  MAY RESULT IN YOUR EVENT BEING CANCELLED.
