CONTRACTOR/WORK REQUESTS

All contractors requesting access to Bellows AFS are required to fill out the attached B-180 application. Once all blocks are filled and background check completed, submit the document to the Visitor Control Center located at Bellows AFS main gate (DSN 259-4493).

Prerequisites prior to contractor badge issuance

Completion of B-180 form

Background check (with no derogatory information)

Valid ID (driver’s license or passport etc)

Not listed on any debarment lists (island wide)

Over the age of 18

Requested work is longer than 6 months
